Appointments

• If you are a new patient, please arrive 20 minutes prior to your visit to complete initial paperwork.

• If you are an existing patient and need to update your insurance or personal information, please arrive 15 minutes early.

• Please remember to bring your insurance card, photo ID, co-pay and a list of any medications that you are taking to your appointment.

• When possible, please schedule your annual complete physical exam at least 6-8 weeks in advance.

• Please be aware that Medicare does not pay for an annual complete physical exam, except for a Welcome to Medicare physical exam within the first six months of Medicare coverage.

• Our providers do their best to stay on time. However, please understand that emergencies and other critical situations do arise and must be addressed with immediately.  If you have been waiting for 30 minutes or more, please let the receptionist or one of our medical assistants know.

*Cancellation & Missed Appointments Policy:

We ask that you please provide at least 24 hours notice to cancel an appointment. If less than 24 hours notice is given or you are more than 15 minutes late for an appointment, you may be charged a $25.00 fee.
